The GEO Group, Inc. (GEO) has gathered correctional facility disturbance information for those
facilities operated by GEO in the United States since 1998 – meeting the definition of “criminal justice
facility”, i.e., a place of total confinement for convicted felons where no felon is allowed off the
premises without the supervision of a sworn peace officer.
GEO has identified forty (40) “criminal justice facilities” that we have operated in the United States
since 1998. Of the forty (40) facilities, nine (9) of those facilities are no longer operated and
managed by GEO. The identified facilities are listed below by state, however, the attached
documents for each facility have been provided in alphabetical order for your convenience.

Sergeant advised that a terminally ill inmate died in the hospital on February 13, 2006.
Upon being released from administrative segregation, an inmate became upset after learning his “partner” would not be
released as well. Inmate declared himself on a hunger strike and threatened to hang himself. The inmate was discovered
an hour later to have committed suicide.

The GEO Group, Inc.

Victim inmate was cut by a ditch bank blade while out on the work line. Initially this was a non-fatal wound. The Inmate
passed away seven days later in a local hospital of a blood clot.
Inmate escaped from officers while admitted to a local hospital. Located two days later hiding under a house in the same
city. Resisted local Law Enforcement SWAT and was shot and killed. (Also see Escape Category below)
Victim inmate was stabbed twice, once each in the chest and back. The inmate passed away in the emergency room of a
local hospital.

Same incident as Inmate Fatality of the same date, above. Cut by another inmate with a ditch bank blade while out on the
work line. A non-fatal wound. Died 7 days later in a local hospital of a blood clot.
Victim Inmate was struck by assailant Inmate in the back of the head with the blunt end of an axe while out on the work line.
Inmate victim of assault was transported to outside hospital for wounds received from a paint scraper. Inmate victim was
treated for eleven stab wounds to the back of his head, one in the neck and one to the shoulder.
Inmate victim of assault by another Inmate was transported to outside hospital for treatment of a fractured jaw. Injury
required surgery.
Two inmates had an altercation in their cell. One Inmate transported to outside hospital for treatment of a possible fractured
jaw. Injury required surgery.

Disturbance in the cellblock over food. Used chemical agents and transferred instigators to another institution.

06/24/2002

Eleven cellblock inmates at the work gate refused to go to work. The incident was resolved without use of force.

10/28/2002

A group of administrative segregation inmates gained access to the dayroom and attempted to take over the unit. CERT
resolved the incident. No serious injuries.

Two inmates ran from the cellblock work line on institutional grounds. Apprehended within six minutes.

08/31/1997

Three inmates cut through several fences. Recaptured three weeks later.

04/09/1999

Two inmates escaped from agriculture work crew. Warning shots fired. Inmates apprehended within six hours.

08/12/1999

Two inmates went over the intake sallyport gate. Both inmates were apprehended within three hours.

07/10/2002

Two inmates ran from a work line on institutional property. Recaptured in six minutes without getting off prison property
and without losing sight of them.
Same incident as Inmate Fatality of the same date, above. Inmate escaped from officers while admitted to outside hospital.
Located two days later hiding under a house in the same city. Resisted local Law Enforcement SWAT and was shot and
killed.
An Inmate exited the compound through the Control Center interlock while walking with two nurses. Another nurse arriving
at the facility at the same time informed a security Sergeant that she thought she saw an inmate off the compound. A quick
search found the Inmate near the Training Building (still on the facility compound) and he was recaptured.
During scheduled count one inmate was determined unaccounted for. A picture bed check was conducted and still
unaccounted for. All proper notifications were made and the inmate was apprehended.
Inmate climbed a fence through razor wire. Inmate was apprehended and returned to the facility the same day.

An inmate was assaulted on the recreation yard by two other inmates (knocked down and kicked in the head) and
transported to the hospital. Inmate returned to the facility the next day and was placed in protective custody pending review
and completion of the investigation. The two inmates were identified and placed in detention.
An inmate was noticed walking around the RTC yard with noticeable trauma to the right side of the face. The RTC yard was
immediately locked down and the inmate was taken to medical. It was determined that the inmate needed further medical
attention and was sent to Maricopa Medical Center. Two inmates were identified as suspects and placed in detention.

Staff noticed that all white and Mexican RTC inmates housed in building 5 were gathering in the yard. All the black inmates
were gathering inside the building. The yard was locked down. All inmates were interviewed and it was determined that the
black inmates had called a meeting to discuss respect issues and all other inmates were unaware of the meeting and exited
the building for safety reasons. The yard was reopened without further incident.
Approximately 60 to 65 Mexican inmates gathered in front of main control peacefully to discuss that they needed to address
and they were not going to work.
Staff noted a large grouping of Mexican inmates in the yard. The yard was locked down. It was determined that the inmates
felt they didn’t receive their state pay. The inmates were given a copy of their bank statement, in all cases the state pay was
correct. The inmates reported to work as scheduled the next morning and the yard was reopened.

Description of Correctional Facility Disturbance

An inmate escaped by climbing the perimeter fence in spite of the no climb wire at the top of the fence and roll of razor wire.
ADC was immediately notified and tactical response and escape chase team implemented. Compromised/damaged razor
wire was replaced.

Approximately fifty-eight Inmates were involved in an incident in the dining hall. Correctional Officers quickly contained the
area. Inmate threw food and kicked doors. Chemical Agents were used to quell the disturbance. There were no injuries to
staff or Inmates and no serious property damage.
During a routine shakedown of a housing area (Unit 2B) forty—three Inmates refused to lock in their cells and became
aggressive toward staff. Chemical Agents were deployed. Inmates complied with orders. There were no injuries and no
significant property damage.
Forty-eight Inmates became unruly in their housing unit when ordered to exit the area for Maintenance to make repairs to
locks on cell doors. The Inmates refused all orders. Chemical Agents were dispersed to control the incident. Order was
restored. There were no serious injuries and no serious property damage.

Three inmates escaped through an emergency door of their living unit. Two of the inmates were recaptured very shortly
after escaping. One inmate remained at large. It is unclear if the at large inmate has been recaptured.
One inmate escaped while in the workman’s compensation office. He was captured within days.
Two inmates assigned to paint recreation basketball court escaped through broken inner fence. Both inmates were
recaptured.
One inmate escaped via the interior perimeter fence while outside during recreation. The inmate was recaptured the same
week.
Five Inmates exited their cells through the alcove and exited out of the housing unit via emergency access to the roof. Two
Inmates were captured between the fences. Three Inmates escaped by climbing down the corner of the Administration
Building. Two of the three Inmates were recaptured.
An Inmate in outside recreation jumped the fence. The perimeter officer fired a warning shot and the inmate surrendered.
One inmate escaped from transport vehicle. Armed Officer left unarmed officer with two inmates. One of the two ran from
the unarmed officer.

5/28/06

Approximately 39 inmates were involved in a disturbance in E-3 Housing Unit. Inmates threw trash, attempted to set
laundry carts on fire, and pulled down dayroom television. Facility CERT team responded and regained control of the dorm
through the use of chemical agents. All inmates were evaluated by medical and no serious injuries were noted. Dorm was
placed on Partial Administrative Lockdown.

An Officer was selected randomly for a urinalysis screening. The test came back positive for cocaine. The officer made a
statement the he has never taken cocaine however the night prior was given medication from a family member for a tooth
ache. The officer states that the kind of medication given was/cannot be identified.
An inmate’s family member contacted the facility to make a complaint regarding a Officer. The complaint stated that the
officer was carrying a cell phone and offering to take photos for family members of inmates. The officer was taken into
custody for questioning by the local police department.

Two inmates were involved in a fight in their cell. Officers responded and stopped them. One inmate required
transportation to outside hospital for fractured left leg.
Two inmates were involved in a fight without weapons, over alleged issues of disrespect. One inmate received significant
injury (possible fracture of cervical spine) that required several days of outside hospital care.
One Inmate was assaulted by three Hispanic Inmates. Victim Inmate was transported to area hospital with possible broken
left ankle. He was admitted to the hospital and required surgery on the ankle.

Two groups of Hispanic inmates (approximately 20) were involved in a dispute that occurred in the gym The dispute was
apparently the result over control of the TV in dayroom. Staff responded and offenders were ordered to stop and all
offenders were place in hand restraints. No force was used. Only minor injuries were received by offenders during the fight
and were treated in unit infirmary.
Four inmates were fighting in a housing unit. This fight escalated to include the entire dayroom (approximately 20
Offenders). Black and Hispanic inmates involved in disagreements over the use of the tables in dayroom. No injuries were
involved and staff controlled the situation.

A female state Inmate –was booked into the CTPVF on 03/16/2001 with no apparent medical issues. On 03/18/2001 the
Inmate was found lying on the floor with blood on the side of her mouth and a strong odor of feces. The Inmate was
transported to outside hospital via EMS where she was pronounced dead as a result of complications of chronic ethanol
abuse.
A male inmate committed suicide by hanging himself with a sheet from tied to a light fixture.

During an Inmate-on-Inmate assault, the assailant Inmate dislocated his shoulder, was taken and admitted into a local
hospital for care.
An altercation between two Inmates resulted in one Inmate sustaining injuries to his shoulder and a cut to his head. Inmate
was taken to the hospital for further evaluation where he remained overnight.
An Inmate found sitting in the dayroom area was observed with blood coming from his right ear and a bump on the left side
of his head. Inmate was taken to the hospital for further evaluation where he remained overnight.

Tobacco. The Officer was arrested in the parking lot and taken into custody.
A female Officer was arrested by San Antonio Police and charged with possession with intent to distribute in a Correctional

A male Officer was arrested and charged with conspiracy to deliver drugs into a correctional facility. The Facility
Administration had requested an investigation into the Officer’s activities. The request led to the subsequent arrest.

An Inmate successfully escaped from the 8th Floor Recreation area by pulling on a chain link fence. In his attempt to climb
down the building he fell and injured his leg. The inmate was later apprehended after seeking medical attention for the

Prison.
Incident involved approximately twenty-seven White and Hispanic inmates in a physical altercation in the dorm area.
A gang affiliated altercation broke out in the B4 housing unit. Verbal order to cease fighting was ignored by all involved. MK9 canisters were used and fighting ceased. No physical force was needed and no significant injuries reported on staff or
inmates.

Approximately fifty to seventy Northern and Southern Inmates became involved in an altercation on the recreation yard. Six
Inmates were transferred via ambulance to St. Mary’s Hospital; Five were treated and transferred off the facility. One
Inmate had emergency surgery for multiple stab wounds. No Staff injuries (Also see Riot or Disturbance below)
A one on one Inmate assault without weapons resulted in one inmate being transferred to outside hospital for treatment for
fracture to orbital and nasal area. At approximately 1935 hrs an officer observed an Inmate near his bunk with a bloody
face.
One Inmate was assaulted by multiple inmates. Transferred to outside hospital for treatment of multiple head injuries.
One Inmate was assaulted by an unknown Inmate or number of Inmates. The victim was transferred to outside hospital for
treatment of lacerations to his face and upper torso. The victim also sustained two fractures to the face and other
abrasions and contusions to the head area.

Inmates refused to eat the noon and dinner meals or to attend afternoon work and program assignments. The following
morning the Inmates returned to eating meals and work and program assignments.

Approximate twenty Hispanic and eight to ten black Inmates were involved in a altercation on the recreation yard. Staff
responded and Inmates obeyed verbal orders to disperse. There were no injuries and no property damage. While
escorting Inmates a staff member found a weapon (sharpened metal object 6” long).

The Inmate population refused to eat the dinner meal. They returned to eating the following morning at breakfast.
10/14/2003
Approximately twenty-one Black and Hispanic Inmates were involved in an altercation. Several inmates were observed
throwing chairs at each other inside the dorm. Three Inmates sustained minor injuries while attempting to stay out of the
way, and were taken to medical for treatment. There were no additional injuries to Inmates or staff.
03/22/2004
With few exceptions, the Inmate population refused to attend the breakfast meal and noon meals. The following morning
Inmate began to eat with the breakfast meal.

During an altercation where one Black Inmate was assaulted by several unidentified inmates a fight broke out in the
housing unit. Approximately fifteen Black and thirty Hispanic Inmates were involved in an altercation. Several Inmates were
observed throwing chairs at each other inside the dorm. One Inmate received a serious head injury and were taken to
medical for treatment.

An altercation between the 60 Black and Hispanic inmates broke out. No serious injuries were reported.

Inmate housed in the facility infirmary on Level 3 Suicide Watch (30-Minute Visual Check) was found lying with his chin
propped on the bunk and blood on the suicide blanket. Inmate was unresponsive. Transported to outside hospital where
he was pronounced dead. Cause of death unknown.
Inmate on Inmate assault in their shared cell. Victim was discovered lying face down in his bunk. Vital signs were absent.
Autopsy revealed victim was strangled to death. Investigated as homicide.
Inmate was found lying on the floor of his cell. CPR was initiated until paramedics arrived. Victim was pronounced dead at
outside Hospital ER. Toxicology report revealed possible Inmate overdosed.
Inmate was found with multiple stab wounds. CPR initiated and inmate transported to outside hospital. Victim died at the
hospital. Incident under investigation.
Inmate was discovered in his cell during count. The hospital pronounced him dead with apparent cause due to an overdose
of an unknown medication.
Officer noticed inmate displaying odd behavior and escorted him to medical. Inmate was then transported to the hospital,
where he was pronounced dead.
Inmate was rushed to the ER with high blood pressure. Was returned to medical, where a nurse found him unresponsive
during a bathing treatment.

Inmate stabbed Inmate in the eye with a homemade knife while they were fighting in the dayroom. The victim was
transported to outside Hospital where he was admitted.
Inmate stabbed Inmate. The victim was being escorted from the shower when aggressor exited his cell and stabbed the
victim in the face and chest area. The victim was transported to outside Hospital where he was admitted.
Inmate assaulted Inmate by picking him up over his shoulders and dropping him with force to the concrete floor. The victim
was transported to outside Hospital where he was admitted for a fractured femur which required surgery.
Inmate stabbed Inmate. Aggressor entered the victim’s cell and stabbed him in the neck. When asked why he stabbed the
victim, the aggressor said “because”. The victim was transported to outside Hospital where he was admitted.

While attempting to subdue an inmate that was hearing voices and had thrown a chair at a security Sergeant,
approximately twenty Inmates began destroying items in the housing area (TV, microwave, countertops, and sprinkler
heads). Chemical Agents were used to disperse the inmates and regain control of the housing unit. There were no injuries
to staff or inmates and the property damage was limited.
Four Inmates were late for breakfast and were not fed. They along with six other Inmates began to make threatening
statements related to starting a riot. Staff contacted the Warden. Upon the Warden’s arrival he and other staff spoke with
each inmate individually. All but two agreed to return to their cells. The two inmates that refused were escorted without
force.
A total of twenty-five staff and twenty-nine Inmates were involved in a disturbance that occurred on Housing Unit 1 “D” pod.
The disturbance was the result of an Inmate gaining control of security keys and let the other Inmates out of their cells.
Inmates destroyed the bulletin boards to fashion weapons and attempted to barricade the dayroom door. The facility’s
Correctional Emergency Response Team (CERT) utilized chemical agents to regain control of the pod. There were no
serious injuries or significant property damage as a result of the disturbance.
An Inmate was able to gain access to the dayroom room his cell. The Inmate gained control of the Officer’s keys and
opened other cell doors. Approximately ten other Inmates exited their cells. The Correctional Emergency Response Team
was activated. Chemical Agents were used to regain control. There were no serious injuries to staff or Inmates and there
was no significant property damage.
11 Inmates involved in an incident in the hall which resulted in stabbing. Chemical weapons were used to contain the
incident, and homemade knives and other weapons were recovered.

Officer was arrested and charged with receiving an escape kit for an Inmate housed on the facility. Investigation revealed
that the Officer along with his wife had developed a plan to aide the Inmate in an escape attempt. Inmate was reassigned
to Mississippi State Penitentiary.
Food Service Cook Supervisor was arrested and charged with possession of marijuana with intent to distribute on facility
property. Confidential information was received that the employee was attempting to introduce contraband into the facility.
A search of subject employee and his property revealed a substantial amount of marijuana.
A facility nurse was arrested and charged with grand larceny. During a search of the nurse’s property it was discovered
that she was attempting to exit the facility with an undetermined amount of medication from the facility’s pharmacy.
Two inmates escaped by breaching the back window of the cell, climbing on top of the administration building, and gaining
access through the front administration area perimeter entrance gates. Both inmates were apprehended later that same
day in Tuscaloosa, Alabama. An Officer believed to have helped the inmates to plan their escape was arrested in
Tuscaloosa after allegedly using her drivers license to obtain a hotel room where the two inmates were located and

Inmate escaped by climbing several perimeter fences. He was apprehended by local authorities on 4/11/05 and transferred
to Mississippi State Prison. No injuries reported.
Two inmates escaped by breaching the back window of the cell, climbing on top of the administration building, and gaining
access through the front administration area perimeter entrance gates. Both inmates were apprehended later that same
day in Tuscaloosa, Alabama. An Officer believed to have helped the inmates to plan their escape was arrested in

altercation and was transported to an outside hospital. Inmate died at hospital. Incident investigated; no charges filed
against second inmate.
Inmate found deceased in cell. Autopsy report indicates death due to accidental drug overdose.
Inmate was transported to an outside hospital where he suffered cardiac arrest and was pronounced dead. Although
preliminary blood tests indicated elevated levels of tricyclics, no official cause of death has been ruled by Medical
Examiner’s Office.
Inmate suffered cervical head fracture and trauma after jumping off a table in his cell and landing on his head. Transported
to outside hospital where inmate later died. Incident under investigation.

Approximately 470 of the 520 Inmates refused to eat the morning meal. The Inmates were complaining about higher
temperatures in the dormitories. The staff was able to resolve the issues and Inmates began to eat with the noon meal.
The incident involved approximately 75 to 100 black and Hispanic inmates in a physical altercation on “B” recreation yard,
and “A3” housing unit. The disturbance was immediately controlled. There were no serious injuries or property damage.
Security personnel notices approximately 50 Hispanic Inmates grouped on one side of the recreation yard and
approximately 25 Hispanic Inmates grouped on another side of the yard. Recognizing this as a potential gang threat,
security staff cleared the recreation yard. All Inmates were escorted to their living dormitory.
Fourteen Hispanic Inmates were involved in a gang related altercation in their dormitory. Staff reacted immediately and
gave verbal orders to cease fighting. All Inmates complied. Only minor injuries to Inmates resulted and there were no staff
injuries. There was no property damage.
The incident involved approximately 27 black and Hispanic inmates in a physical altercation in “B1” housing unit. Only
minor injuries to Inmates resulted and there were no staff injuries. There was no property damage.
The incident involved approximately 50 White and Mexican National inmates in a physical altercation on the recreation
yard. The disturbance was immediately controlled. Three inmates were transported to North Kern State Prison for
additional medical treatment.
Incident involved approximately 225 inmates gathered on the "A" side recreation yard who refused to disperse. GEO and
CDC staff responded to the yard and the Warden spoke to the group. The inmates complained that they had some respect
issues with some staff and wanted some resolutions. The Warden addressed the group and all inmates agreed to return to
buildings. The facility was placed on modified lockdown with dayroom activities only. Approximately 40 inmates
representing the different ethnic groups met with the Administrative staff in the visitation room. After hearing all, there was
a consensus that a Lt. and two officers were the root of the problem. It was recommended that these staff members be
transferred to another facility for the safety and security of the facility. This was a very passive resistance.

A Correctional Officer confiscated an item of contraband from Inmate. Inmate became upset and battered Officer with a
closed fist from behind in the head causing a laceration. The Officer was transported to the hospital. The laceration
required six sutures and the officer was kept overnight for observation purposes
Three Correctional Officers were assaulted by an inmate with a weapon. The assault began on one officer after an
argument between the two. The second officer attempted to assist the first officer and became the center of the inmate’s
aggression. A third officer entered to assist in the exit of the two involved officers. All three officers were transported to the
hospital for definitive care. The first and third officers were released, while the second officer remained in the hospital and

A disturbance took place with over two hundred participants that resulted in one staff member killed (see Staff Fatalities
above), one inmate injured, and significant property damage. The disturbance was contained to the inmate housing areas
and quelled six hours later. The facility was placed on lockdown status pending completion of an investigation.
There was an inmate fight involving ten inmates. The inmates refused to stand down from their actions and chemical
agents were utilized to gain compliance. The facility was placed on lockdown status, resulting in a major disruption of
facility operations. There were no significant injuries or property damage resulting from the incident.
As the assault on the three officers mentioned above (see Battery on Staff above) was taking place the housing area
across the corridor became unruly and was encouraging participation in both the housing area and the housing area across
the corridor. Security staff dispersed chemical agents and all inmates returned to their assigned cells, with no significant
property damage or injuries sustained.
The facility population was put on Institutional Lockdown status, inmates in 4 housing pods refused to comply with facility
orders to lock down and became disruptive. The facility CERT was assembled and quelled the situation with chemical
grenades.

A Correctional Officer was arrested after the local Sheriff’s Department conducted an undercover operation related to the
introduction of Narcotics into the facility. An Inmate was given a marked $100.00 bill and successfully exchanged this bill
for 4.8 grams of Marijuana from the Officer The $100.00 bill and illegal drugs were recovered immediately and the Officer
was taken into custody.

attempting to close the door when approximately fifteen Inmates rushed the door in an attempt to gain access to the
corridor. Five Inmates forced their way into the corridor. One Major Use of Force occurred while attempting to gain control.
The remainder of Inmates complied with verbal orders and entered the dorm without further incident. There were no staff
or Inmate injuries.
A fight broke out in a medium custody dorm between two Inmates. The fight escalated to involve approximately thirteen
more Inmates. Staff arrived and ordered the inmates to cease fighting. All Inmates complied.

and five white Inmates. When ordered to stand down by staff, Inmates complied without further incident.

whites taking part. Supervisors arrived, instructed Inmates to lie face down and all Inmates complied. Three Inmates
received minor medical attention.

During religious services turn in, approximately sixteen white and eight black Inmates engaged in a verbal confrontation.
Officers intervened, each group was separated. Approximately 30 minutes later, one black and one white Inmate engaged
in a confrontation in a medium custody dorm. Approximately 5 minutes later, a fight broke out between one black and one
white Inmate in another dorm. The fights were contained.
03-31-2000
A group disturbance occurred in a medium custody dorm involving approximately twenty Inmates. Orders were ignored to
cease fighting. Chemical Agents were dispersed. The fighting ceased and the Inmates were decontaminated.

A racial disturbance involving 48 black and Hispanic medium custody inmates occurred in their dormitory. Staff responded
quickly and dispersed Chemical Agents to gain control. There were no serious injuries to inmates and property damage
was minimal.

A fight involving twelve medium custody Inmates occurred in a dorm. The altercation appeared to be divided along racial
lines. The confrontation was quelled by officers.
02/07/2003
A fight between approximately six medium custody inmates took place in their dormitory. Possible racially motivated. Two
inmates received serious injuries. One inmate with possible fractured mandible and the other with possible nasal fracture.

Approximately ten medium custody inmates (five black, four white and one Hispanic) were involved in a physical altercation
in their dormitory. They used tables and chairs as weapons. Only minor injuries were noted. Property damage was limited

An Inmate assault victim was found unresponsive on his cell floor by an officer during the noon count. He was taken to the
local hospital for injuries he received. A short time later the inmate was placed on life support. The inmate was later taken
off life support and passed away. The assailant inmate later pleaded guilty to a charge of Murder and received a Life
Without Parole sentence in Oct. 2003.
Correctional staff found an Inmate assault victim unresponsive in his bed with blood on his face and a cord around his neck.
His cellmate later confessed to the homicide. Cause of death was asphyxia due to strangulation.

As the result of an Inmate on Inmate assault Victim Inmate was found unresponsive inside of his cell lying on the floor
between the desk and bed. Inmate was taken to the hospital and admitted due to injuries.
Victim Inmate was found assaulted inside of his cell, unresponsive to staff, and bleeding from several different lacerations
to his head. Admitted for observation. The victim Inmate had been assaulted by four assailant Inmates.
An Inmate victim was assaulted by another inmate with a sharpened instrument. Victim Inmate received a laceration to his
neck area and was taken to the local hospital where he was admitted.
Victim inmate was found unresponsive on his cell floor by an officer during Noon count. He was taken to the local hospital
for injuries he received. A short time later the inmate was placed on life support. It was later learned that the Inmate was a
victim of an assault. The assailant inmate later pleaded guilty to a charge of Murder 1 and received a Life Without Parole
sentence in Oct. 2003 (See Fatality Section Above).
An Inmate victim was assaulted by another inmate with a sharpened instrument. Victim Inmate received a puncture wound
to his upper right body. He was taken to the local hospital where he was admitted.
An inmate-on-inmate assault resulted in the Inmate victim’s jaw being broken in two places. Victim inmate was transported
to and admitted to a local hospital for care. Victim inmate was later transferred to a hospital in the nearby metroplex area.
Officers found an Inmate victim on the floor in his cell in a pool of blood. Inmate victim was transported by ambulance to the
local hospital where he was admitted and treated for internal injuries. The Inmate assailant confessed to the assault and
evidence was turned over to the local authorities for processing.
While in the recreation yard two inmates collided while playing a game. One inmate received injuries to his eye which
required surgery. It was not determined whether the incident was an accident or intentional.
An inmate was assaulted by three other inmates with a sharpened weapon. The inmate received a laceration to his liver.
An inmate was assaulted by one other inmate receiving multiple lacerations to the eye area. The inmate was admitted to
the hospital for observation and released the next day.

Approximately 16 inmates were involved in an altercation. The Native American and Hispanic inmates were confronting the
Black inmates regarding a theft that had taken place within the facility. The disturbance was quelled and inmates returned

The GEO Group, Inc.

At the time of this incident inmates were being served meals in the chow hall. The Native American group was dissatisfied
with treatment and felt that they were being denied religious/cultural programming. Twenty-eight inmates were identified as
instigating the incident with over 100 becoming involved in the disturbance which originated in the chow hall and traveled to
the hallway immediately outside the chow hall. The inmates possessed various weapons. Thirteen staff members and two
visiting NMCD STG officers received minor injuries. The situation was contained by on-duty staff within 30 minutes.
Approximately 57 inmates were refusing to return to their cells after complaining about the noon meal. Chemical agents
were administered and the disturbance quelled. All inmates returned to their cells.

Thirty four Inmates participated in a group disturbance during which four inmates assaulted five other inmates. The facility
Correctional Emergency Response Team was called in to quell the disturbance. Only minor injuries were noted.
Nine Inmates were involved in a disturbance in their housing unit. The disturbance was the result of a disagreement over a
board game. Only minor injuries were noted. No force was needed to stop the altercation.
At least five Inmates were identified as being involved in an Inmate work stoppage/disturbance. There were no injuries or
property damage.
5 Inmates participated in a dining hall fight which was contained at the crash gate.
28 Inmates were involved in an altercation, throwing garbage cans and hitting each other with chairs. One inmate sustained
a laceration to his clavicle and on his fingers and wrist. A bag of leafy green substance was recovered.
A disturbance at the facility occurred that dictated a lockdown of all inmates so that a search could be conducted of the
entire facility for contraband.
Approximately 35 inmates were involved in an altercation in the dorm. After several verbal orders to cease fighting were
ignored, two one second bursts of a gas gun were sprayed. Inmates ceased fighting and were separated.

Two rival gangs, the Southerners and Fresno Bulldogs, were fighting on the recreation yard. Approximately 30 inmates
were involved in the altercation. All inmates involved complied with verbal orders to cease fighting.

Two inmates were involved in an altercation with 15 unidentified Hispanic inmates on the inner recreation yard. All inmates

Approximately 15 inmates were causing a disturbance (Southsiders and Fresno Bulldogs). Verbal orders were given to
cease fighting and they complied.
Approximately 17 inmates (Northerners and Southerners group members) began causing a disturbance. Verbal orders

An inmate disturbance began with 20-30 inmates not complying to the dress code and grooming standards. The inmates
were non-compliant and refused to return to their assigned unit until they were compliant. The inmates became aggressive
and rushed as several officers. Once the disturbance was under control the inmates were directed back to the housing
area. Instead the inmates moved into various units with the intent to damage the building, windows, doors, computers and
other facility property. The facility was put on lockdown and the disturbance was contained.

A fight between two inmates erupted in the dorm. One Inmate struck another Inmate in the face with a combination lock tied
to a sock. The Inmate that was struck was transported by ambulance to an outside Hospital where he underwent surgery to
repair a fractured bone in his face.

8/6/04

Several inmates assaulted 14 inmates during a planned assault. The assailants were identified as members of the “Paisas”
and the victims as members of the “Surenos.” The assaults occurred simultaneously at several different locations
throughout the facility and the assailants used sharpened instruments, can lids, and combination locks in socks as weapons
to assault their victims. Eight of the 14 inmates assaulted required outside medical treatment. Of the 8, two were treated
and released, two were kept overnight for observation, and one was air lifted to a hospital in Lubbock, TX but was returned
the following day. There was no force used by staff and no injuries to staff. The incident resulted in a 3-day lock down of the
facility.

The entire inmate population (approx. 2100) engaged in a food strike and refused to partake of any meals. The chief
complaint of the inmates centered on changes related to food preparation. The facility initiated a lock down and identified
the instigators. While conducting interviews on a dorm the inmates assigned became disruptive and refused orders given
by staff. The inmates refused to allow any inmate to be removed from the dorm for interviews. The CERT team was
assembled for a show of force and the inmates retreated to the back of the dorm and began verbally challenging staff.
Chemical agents were deployed and the CERT entered the dorm and restrained the inmates. Once the dorm was
decontaminated all the inmates were placed back into the dorm. After interviews were conducted to address the inmates
concerns normal operations resumed without incident.
Approximately 150 inmates in a housing dormitory refused to rack-up due to complaints about a correctional officer.
Compliance was obtained through a show of force. The housing unit was locked down for four days while administrative
staff interviewed the inmates.
Approximately 14 inmates began flooding the special housing units by using objects to obstruct the drainage in their toilets
because they were scheduled to be transferred from the facility. No use of force was issued.

Approximately forty Inmates (level 2 Mexican Nationals) began fighting in a dayroom. Investigation revealed that they were
from rival gangs in Arizona (Border Brothers/Paisa). No serious injuries were sustained.
Ten Mexican National (medium custody) Inmates began participating in a work stoppage and hunger strike. This action
was due to Level 2 minimum custody Inmates arriving. These Inmates are not housed at the same facility in Arizona.
Through interviews, the incident was resolved and all Inmates resumed working and eating on 4/12/04.
All Black Inmates housed in C Building (minimum custody) began a work stoppage because a Black Inmate from their
housing unit had been placed in detention. Investigation revealed that four of these Inmates had threatened the rest with
physical harm if they continued to work. These four were placed in the Special Housing Unit and the rest returned to work.
Forty-three Mexican National Level 3 (medium custody) Inmates began destroying property in the dayroom by breaking
windows, TV’s, toilets, etc. CS Chemical Agents were deployed and the disturbance was quelled in less than 2 minutes.
No serious injuries were sustained. Property Damage was minimal. The incident was a result of the Inmates being angry
over being moved from Arizona to Texas.
Two hundred eighty eight Level 3 medium custody Inmates participated in a group work stoppage. They stated that the
work stoppage was due to them being transferred to Texas. Investigation was conducted and all Inmates were interviewed.
They agreed to return to work on 5/25/04.
Two hundred twenty-five Level 3 medium custody Inmates were involved in a disturbance resulting in property damage. CS
and OC chemical agents were deployed and the disturbance was quelled in approximately 4 minutes. No inmate or staff
injuries were sustained. The housing area was locked down for 6 days.
Approximately 16 inmates involved in beating and kicking doors in detention unit. OC aerosol spray used and inmates put
on property restriction.
A fight between approximately 50 inmates erupted on the recreation yard leaving 2 injured. Immediately after, staff
responded to a fight in a dorm, leaving two more injured.
Approximately 340 inmates refused to participate in count. Chemical agents were utilized. Inmates then complied with
orders to count. No injuries involved.

One Inmate was assaulted by two Inmates. The victim suffered several broken teeth and a possible fractured jaw. Inmate
was transported to outside hospital for treatment and observation.
One Inmate informed a Security Supervisor that he had been attacked by his cell mate and possibly raped. The Victim
Inmate stated that the attacker had chocked him unconscious and raped him. The victim Inmate was transported to outside
hospital for evaluation and observation.
An Inmate was assaulted by aggressor Inmate in the dayroom. The assault went unnoticed by staff, and Victim Inmate
reported that he fell and hit his head. Further investigation revealed that the assailant struck the victim in the head with the
handle of a push broom. Victim suffered broken teeth and contusions to the head.
Two Inmates were involved in a fight. After being hit in the face by a fist, one of the inmates fell (face first) and hit his head
on the floor.
Two Inmates were involved in a fight in their cell. One Inmate was knocked unconscious. Victim Inmate was transported to
the hospital and diagnosed with a concussion. Admitted to outside hospital for observation.
Two inmates were involved in a fight in the dayroom. One Inmate grabbed the head of a push broom and struck the other
inmate. The Inmate that was struck was knocked out. Transported to outside hospital for treatment and overnight
observation.

Eight rival gang members (Bloods and Forest City Boys) were involved in a disturbance at the morning meal following a
disagreement between two of the Inmates the night before. Staff responded and gave verbal orders to stop fighting. The
Inmates complied. There were no serious injuries or property damage.
Nineteen Inmates began fighting in 2 E day room. One Inmate possessed a weapon (sharpened fence tie) and used it to
stab two other inmates resulting in superficial wounds. Injuries did not require off-site hospitalization. The incident was
gang related.

An Inmate committed suicide by hanging himself from the air vent with a laundry bag string tied around his neck.

04/27/2002

During a search an inmate was observed placing something in his mouth and was instructed to spit it out. The Inmate
refused and was uncooperative. This lack of cooperation escalated and resulted in the use of Chemical Agents. Upon
medical examination for a post use of force physical the nurse discovered that the inmate had a weak pulse and was not
breathing. CPR was started and EMS notified. Paramedics removed a zip lock bag containing twenty-nine balloons of
suspected narcotics from the inmate's throat.

An Inmate attacked a Security Lieutenant during a routine escort inside the facility. The Inmate struck the Lieutenant
knocking him to the ground. While on the ground the Inmate began kicking the Lieutenant. After the Inmate was subdued
the victim was transported via EMS to an outside hospital for treatment. The Lieutenant required reconstructive surgery to
repair the right orbit and his jaw was wired shut to prevent movement.

A registered Nurse was arrested for Medicaid Fraud.

8/8/98

During routine searches of personnel entering the facility a Correctional Officer was found in possession of 32 grams of
marijuana, two packs of cigars, and a bottle of cologne. The Officer was arrested by the South Bay Police Department and
charged with Possession of a Controlled Substance, Conspiracy to Traffic a Controlled Substance in an Institution, and
Introduction of Contraband into a Correctional Facility.
An Off-Duty Sergeant was arrested by the Palm Beach County Sheriff’s Office and charged with Attempting to Elude Police,
Reckless Driving, and Speeding. He was arrested in the facility parking lot and taken to the Palm Beach County Jail.
A Counselor was arrested in the parking lot of SBCF. The Counselor had accidentally run into another employee’s vehicle.
The Counselor left a note on the car, and then left for lunch. Later when the Counselor refused to give the owner of the car
he hit his driver’s license information she called the police department and reported a “hit and run.” SBPD ran a check on
the Counselor and discovered that his license was suspended and he had outstanding warrants in other counties. The
Counselor was arrested and removed from the property by SBPD.
A Correctional Officer and a Food Service Worker were both arrested for accepting money from an Inmate in a conspiracy
to bring narcotics into the institution. The money was recovered and no narcotics were introduced into the facility.
A substance abuse counselor was charged with possession of marijuana over 20 grams, smuggling contraband into a
prison and possession of marijuana with intent to sell.

Two inmates on the horticulture area cleaning crew breached a secure door, climbed over an exterior perimeter fence, and
left the facility on foot. They were recaptured within in 2-1/2 hours.
Civil Commitment walked away from a mental health appointment while awaiting transfer to South Florida State Hospital.
He was apprehended in 30 minutes.

Ten black and 30 Hispanic inmates were fighting in the recreation yard. Resulted in 20 inmates being relocated to the
Special Management Unit and A3-B Dorm locked down. No serious injuries or property damage.
Fifty-Eight (58) inmates were placed into the SHU for refusing to work in food service. The inmate population responded by
engaging in a disturbance which caused $20,000 dollars in damage. The disturbance lasted approximately three hours and
ended with two minor inmate injuries.
A disturbance occurred between the Black and Hispanic inmates. It stemmed from a prior fight between the two races
earlier in the day in which two Hispanic inmates and one Black inmate were placed into the SHU.
Approximately 300 Federal Prisoners assigned to work in UNICOR (Federal Industry Program) failed to report for work.
Mass interviews resulted in complaints related to the quality of food and medical attention. Facility was locked down.
There were no assaults, property damage or injuries. Normal operations resumed eight days later.
Several white inmates, responding to an earlier assault on Whites by Paisa inmates over a suspected drug deal, began
assaulting several Paisa inmates creating an institutional disturbance.

An inmate, utilized visitation to facilitate his escape from the low custody facility. At the conclusion of his visit, he walked
out of visitation with a group of visitors. Upon investigation, it was discovered that he had been making arrangements for
this escape for several days.
An inmate escaped from TCI by hiding in the institution’s trash compacter. Inmate Orozco waited for the trash compacter to
be picked up and delivered to the local landfill. Once he arrived at the landfill, he was spotted running from the recently
deposited trash. A landfill worker spotted the inmate running toward a small local town and immediately notified TCI. The
inmate was apprehended by TCI staff a few hours later.

08/15/1999
A Racial group disturbance occurred (number of participants unknown). All Inmates complied with orders to stop their
actions. Chemical Agents were not used. Two Inmates sustained minor injuries.
09/20/1999
A racial group disturbance occurred, in housing unit #3, dorms A & C. Unit 3C complied with orders to stop fighting 3A did
not comply, chemical agent were used. Inmates complied with orders. Five Inmates were injured and 2 Officers were
injured. Injuries were not serious.
02/20/2001
Due to several minor Inmate disturbances involving suspected members of the security threat group a Facility wide
shakedown/lockdown was initiated.
04/06/2001
A Disturbance involving thirty six Inmates resulted in the use of Chemical Agents to quell. There was minor property
damage to the day housing unit day room.
05/15/2001
Due to the assault of an Officer without a weapon, Institutional Lockdown was initiated. While the facility was locked down
a group disturbance occurred. Chemical agent was dispersed to gain control.
10/18/2001
A Disturbance involving forty-one Inmates resulted in an Institutional Lockdown. There was minor property was damaged.

A Disturbance involving fifty-two Inmates resulted in the Use of Chemical Agents. There were minor inmate injuries, and
limited property damage.
A Disturbance involving thirteen Inmates resulted in an Institutional Lockdown. There were no serious injuries and no
property damage.
A Major Disturbance involving thirty-five Inmates resulted in an Institutional Lockdown. There was both minor property
damage, and minor injuries.
A Major Disturbance involving twenty-five to thirty Inmates resulted in minor property damaged. Weapons (fire
extinguishers) were used. There were no staff use of force, nor were there any injuries. Possible gang related incident
between Texas Syndicate and Raza Unida.
A Group disturbance, involving twelve Inmates, property was damaged, no serious injuries.
A Disturbance involving forty-eight Inmates resulted in an Institutional Lockdown. Property was destroyed and there were
minor injuries.

Description of Correctional Facility Disturbance

An Inmate escaped by walking out the front through Administration building and running from the perimeter patrol officer.
The Inmate was captured before he got off facility property.
An Inmate escaped by climbing over the perimeter fence. The escape occurred just prior to sunrise during heavy fog and
rain. The Inmate was capture 48 hours later by Facility Staff.
